There's 2 ways to make referrals to the CALMHS Service;
Self referral
| You can make a self referral by completing a Self Referral Form or by contacting one of our team. Once we've received your completed form we'll contact you to arrange a date for an assessment within 4 weeks. |

Other referral
Healthcare professionals can make referrals on someone's behalf. We accept referrals from;
Once we've received a referral we'll make contact within 48 hours by telephone and offer an assessment appointment within 4 weeks. |
